ESI Corporation – Insured Persons Quota for MBBS/BDS
If either of the parents are payers of Employees State Insurance (ESI), then you can claim ESI Insured Persons Quota for MBBS/BDS through NEET-UG. To claim this, you must get certificate by applying online.
ನಿಮ್ಮ ಪೋಷಕರು ಕಾರ್ಮಿಕರ ರಾಜ್ಯ ವಿಮಾ ಪಾವತಿದಾರರಾಗಿದ್ದಲ್ಲಿ, ನೀಟ್ ಮೂಲಕ ಎಂಬಿಬಿಎಸ್/ಬಿಡಿಎಸ್ ಪ್ರವೇಶ ಪಡೆಯುವಾಗ ಇಎಸ್ಐ ಚಂದಾದಾರರ ಕೋಟಾ ಕೋರಲು ಕಡ್ಡಾಯವಾಗಿ ಆನ್ಲೈನ್ ಮೂಲಕ ಅರ್ಜಿ ಸಲ್ಲಿಸಿ ಪ್ರಮಾಣಪತ್ರ ಪಡೆದುಕೊಳ್ಳಬೇಕಾಗುತ್ತದೆ.
REQUIRED DOCUMENTS
- 2 Passport Size Photographs of Insured Person
- Copy of ESI Card
- Copy of the following Documents:
- Wage Slip for October 2018
- Certificate of Attendance of Insured Person (from 01.11.2017 to 31.10.2018)
- Detail of EPFO Payment (the employee)
- Copy of Affidavits for female candidates (as annexure 4 & 5)
- 2 Passport Size Photographs of Student
- Copy of NEET-2019 Admit Card
- Copy of Proof of Residence
- Copy of SSLC Mark Card
- Copy of II PUC Mark Card
- Copy of SC/ST/OBC/PwD Certificate (Central Govt Format)
- Copy of EWS Certificate (Only for General Merit below 8 lakh income)
